New York State has recently enacted the Compassionate Care Act, making Medical Marijuana available for certified patients with serious conditions. Our doctors work closely with patients who have qualifying conditions to determine necessity and eligibility.

A common misconception about medical marijuana is its side effects. Unlike those who use the drug recreationally, patients who receive medical-grade marijuana will not experience a “high.” Because medical marijuana contains CBD, a natural antioxidant and the compound thought to be beneficial for patients, and none of the psychoactive ingredients found in recreational-use marijuana. 

IF YOU ARE A RESIDENT OF NEW YORK STATE AND HAVE BEEN DIAGNOSED WITH ONE OF THE QUALIFYING CONDITIONS, YOU MAY BE ELIGIBLE TO RECEIVE MEDICAL MARIJUANA.


Additional Patient Information

If you are a New York or New Jersey resident, you are potentially eligible for medical marijuana if you have been diagnosed with a specific severe, debilitating or life threatening condition that is accompanied by an associated or complicating condition. By law, those conditions are:

  • Cancer

  • HIV/AIDS

  • Amyotrophic lateral sclerosis (ALS)

  • Parkinson's disease

  • Multiple sclerosis

  • Spinal cord injury with spasticity

  • Epilepsy

  • Inflammatory bowel disease

  • Neuropathy

  • Huntington's disease

  • Chronic Pain

  • Opioid Replacement

  • The associated or complicating conditions are cachexia or wasting syndrome, severe or chronic pain, severe nausea, seizures, or severe or persistent muscle spasms. 

New York / New Jersey Discrepancies

Your state of residency determines your options. We can discuss your NJ is plant product whereas NY is vape, tincture or pill form.

HOW DO I QUALIFY?

STEP ONE:

Send an e-mail to team@parkavenuemedicine.com to request the Medical Release form  to give to your doctor  in order to authorize them to send us your supporting medical documentation.

If documentation is not easily available, or you're not certain of your eligibility, you should schedule a Medical Visit by calling our office at (212) 371-8460.

STEP TWO:

Once we receive your medical records from your doctor, our staff will call you to set up an appointment with one of our Board Certified and Medical Marijuana Registered Practitioners. 

You will have two appointments:

  1. Pre-Medical Marijuana Appointment: to go over your medical documents and discuss if Medical Marijuana is right for you. This visit helps to establish a legitimate patient/physician relationship. Please note this is a medical screening visit only, to help determine ones eligibility. The office accepts most Insurances for this visit.

  2. This visit is for patients who are Eligible for Medical Marijuana: Eligible patients will obtain a certification at this visit along with a recommendation for the dispensation of Medical marijuana.  This Certification visit is not covered by insurance and is at the self-pay rate.  This Certification can be valid up to 12-months. 

STEP THREE:

Once you receive your certification you will be able to register yourself for a Medical Marijuana Card on the New York State Department of Health Website, this card must be shown when buying medical marijuana at a dispensing facility. ​
