  • Seongjoong Lee (Doctorate of Physical Therapy, Montana University 2022)

    • Dr. Seonjoong Lee has been practicing for over 10 years as a Physical Therapist. He specialized in soft tissue release, therapeutic exercise, orthopedics and sports therapy, outpatient style orthopedic home health as well to help people optimize their movement health and recover from injury.

       

      Dr. Seonjoong Lee earned his bachelor’s degree in physical therapy from Sahmyook University in South Korea. He then obtained his Doctor of Physical Therapy degree from Montana University. Dr. Lee has various Sports and Orthopedic outpatient therapy experiences in New York. He also trained in Multiple Clinical Manual Therapies, Sports Taping, and Functional Mobilization skills, IASTM (Graston Technique), Cupping.

       

      He values evidence-based practices (EBPs) and modern clinical approaches when evaluating and managing patients' pain and physical problems.

      He loves helping patients get better every day by establishing specific and targeted exercise and training routines that prevent trauma and improve their daily lives as well as treatment.

       

      He is also studying Master of Acupuncture at New York College Traditional Chinese medicine, driven by a desire to expand his education beyond the field of Western Medicine.

       

      In his spare time, he enjoys spending time, lifting weights at the gym, running, traveling. He loves spending quality time with his family.

  • Jeffrey Yellin (Doctorate of Physical Therapy, Stonybrook University 2006)

    • Dr. Yellin has been certified as a strength and conditioning specialist through NSCA, as a sports performance coach through USA weightlifting association, and as an active release technique practitioner through ART.

      He was an original 10 partner of one of the largest outpatient physical therapy practices in the northeast, helping the company grow from 6 to over 170 locations. 

      Jeff has also assisted in the rapid growth and compliance of various healthcare companies nationwide providing consultation and speaking services to clinicians and upper management. 

      He is widely experienced in working with professional athletes across all the major sports leagues and fighting federations for rehabilitation, injury prevention,  and strength and conditioning services.  A renowned leader in the field of physical therapy, Jeff's expertise can also be found in various top lifestyle publications including Bustle, SELF, EliteDaily, and Shape Magazine.

Home Physical Therapy

We offer in-home physical therapy sessions throughout the United States, serving all the neighborhoods within our reach. Our aim is to make physical therapy as convenient as possible by coming to you.

Our Physical Therapists are available seven days a week for private physical therapy sessions in the comfort of your own home. With appointments from 6:30 am to 8:30 pm, we make it easy for you to complete your full course of care without any hassle. Our focus is on providing in-home therapy, so you can receive the treatment you need in a familiar and comfortable environment.

Our goal is to ensure that each patient is consistently treated by the same therapist throughout their entire course of care. Although scheduling priorities may occasionally require adjustments, we make every effort to inform you in advance about who will be seeing you for your appointment.

Individualized Programs

Rehabilitation after an injury or surgery is often overlooked because some people prefer to heal on their own, without experiencing any pain. However, at PARK AVENUE MEDICINE & SURGERY, our licensed clinicians design personalized treatment programs to help reduce pain and provide relief. In some cases, such as when recovering range of motion after a shoulder or knee injury, pain may be experienced during therapy. It is important to communicate any pain or discomfort to your physical or occupational therapist, so they can make necessary adjustments to your treatment plan.

Types of Physical and Occupational Therapy Treatments

At PARK AVENUE MEDICINE & SURGERY our Physical and Occupational therapists employ various treatments to address our patients' needs. Cryotherapy may be used to reduce swelling, parallel bars may be utilized for gait training, electrical nerve stimulation may be used to relieve pain, and massage may be employed to decrease scar tissue. Even after completing your treatment program, it is not unusual to experience a "flare up." In such cases, it is important to communicate with your physical therapist, who can advise on whether additional treatment is necessary or suggest adjustments to your daily routine and activities.
